It's finally time to get your new appliances delivered and Creative Appliance Gallery wants to ensure this process goes as smoothly as possible.  As such, we ask you to take a few minutes to fill out this questionnaire and provide us with important information to protect your investment.  Should some information change before delivery, you can always fill up a second questionnaire and as always, do not hesitate to contact us if you have any questions or concerns.
